Steps to Redeem

1. Fill in this Refill Voucher

2. Include a Demand Draft (DD) of Rs. 149/- per bottle towards your Return Shipping and bottle Handling for your Free Refill. Demand Draft Favoring “Home Shoppe Direct Merchandising Pvt. Ltd.” Payable at Mumbai, India” only will be accepted.

3. Mail/courier this form along with the required payment and a valid photo ID and the address proof copy to:  Home Shoppe Direct Merchandising Pvt Ltd. Trade Center, Level 1 Bandra Kurla Complex, Bandra East, Mumbai-400051 India.

4. Upon receiving your duly filled claim form and only upon full realization of your payment, within 15 working days, your refill claim request will be processed. Upon successful processing, a Refill  Confirmation Voucher with a special RMA number ( Refill Merchandise Authorization number) will be mailed to you through ordinary post/mail at your home address as provided by you in this form.

5. Please place the Refill Confirmation voucher (with the RMA number) along with your empty bottles in the self addressed Refill Bag which came along with this Refill Voucher and mail/courier it to our Redemption Center.

6. Your FREE REFILL bottle will be dispatched to you within 15 working days from the date of receipt of the above to your home address as provided by you in this form.

 
Terms and Condition

1. The refill claim is valid only, if the customer, successfully registered within free Refill promotion claim, as mentioned on the product packing, within 7 working days of their purchase, and successfully obtained a claim Registration ID number from Customer Care at 022-6511-3355,

2. The Customer’s Name and Number should exactly match with the Name and the Address provided at the time of purchase and at the time of obtaining the Registration ID number.

3. The product is meant for a customer’s personal and home usage only (1 person usage only) and the promotion is valid and limited to for home residential usage only. Usage for any other purpose, including, but not limited to, commercial usage is strictly prohibited and invalidates the claim.

4. From the date of their purchase, for every successful quarter of continuous & uninterrupted usage of the product (Jawed Habib Total Therapy), only one Free Refill can be claimed with a maximum of 4 refills in the given year from the date of their purchase.

5. Only Original Refill Claim forms and only original Refill confirmation Vouchers will be accepted.

6. It is the sole responsibility of the customer to full fill the necessary terms and conditions and to allow all the steps required to determine his eligibility of this Free Refill Claim and for nay such failures, or refusal or denial of the claim as a resultant of such failures, “Home Shoppe Direct Merchandising Pvt Ltd” or its distributors, or its retailers, or its selling partners and/or any associate cannot be held responsible.

7. “Home Shoppe Direct Merchandising Pvt Ltd”, Cannot be held liable or Responsible for any lost, misdirected, incomplete, illegible, late or mutilated Refill claim forms, Refill confirmation voucher, self address refill bags or nay other mail/courier or nay communication sent by/to the customer.
